The Emergency Service for Carers

Registering

 

Once a carer has provided us with their basic contact details, an acknowledgement letter is sent to them explaining that a member of the Care Team will contact them within a certain period of time to arrange a suitable appointment with the carer and cared for.

A Senior Care Support Worker will arrange a home visit to complete:

  • The full registraion form,
  • The emergency care plan,
  • The risk assessment.

All this information will then be entered on to the CERS database. Carefirst details included.

When that has been completed the carer will be issued with their own individually numbered emergency card.   The card will have a message to say that the carrier is a carer and that someone relies on them.  The emergency contact phone number will be on the card with instructions of what to do in an emergency.  There will be no personal details on the card.  The emergency contact number will be on the card with instructions of what to do in an emergency.  The emergency line will be staffed 24/7 365 days a year by a member of our Care Management Team.
