The Emergency Service for Carers
When the carers details are received the Care Management Team will contact the carer to arrange a convenient time for a Senior Care Support Worker to visit them at home to complete:
All this information will then be entered on to the CERS database. Carefirst details included.
When that has been completed the carer will be issued with their own individually numbered emergency card. The card will have a message to say that the carrier is a carer and that someone relies on them. The emergency contact phone number will be on the card with instructions of what to do in an emergency. There will be no personal details on the card.
