Emergency Plan

During the visit the staff member will complete a Needs Assessment and Risk Assessment to gain the information needed to complete the Care Plan.  An Emergency Action Plan will also be completed which will include who, if necessary, should be contacted in an emergency.  CERS will also liaise with other relevant professionals in the development of the service and care plans where this is needed.

Emergency Call Outs:

A member of the Care Team will go into the carer's home to take over their caring responsibilites.  They will undertake the tasks carried out by the carer as detailed and agreed in the needs assessment and care plan. A personal care plan will be agreed with the person/s with care needs and may include the following:

Basic Tasks: Support with

* Bathing

* Personal Care

* Mobility and Transfers

* Dietry requirements at meal times, including preparation support when required

* The management of continence of bladder and bowel

* Administration of prescribed medication
