The Emergency Service for Carers
The scheme is led by the Specialist Service Manager. There are two branches of the service:
The Outreach Team who: Publicise, promote and market the scheme across the City
The Care Team who: Visit the carer in their own home to complete their full registration to the scheme including drawing up their Emergency Action Plan and completing a comprehensive Needs Assessment, Risk Assessment and Care Plan. Provide Emergency respite care for the Cared for when needed and liaise with other agencies as and when needed. e.g social services, doctors etc...
