- To provide a back up service to carers to ensure that the person they care for is given support in the case of an emergency.
- To register carers and to draw up a detailed contingency plan with the carer and cared for
- To carry out a needs assessment and risk assessment
- To provide a home-based care service for upto 48 hours (and upto 72 hours during Bank Holidays) when necessary
- To liaise with care management teams from both Adults and Communities and Children's Directorates when continuing care is needed
In addition to this, our service objective also includes:
- As well as ensuring that the person is given support, such a service would provide peace of mind and therefore reduce stress levels of carers.
- Identify hidden carers who don’t necessarily think of themselves as carers.
- To reach carers who don’t currently access any relevant services or organisations.
- Add value to carers services.
